Retiree Dental Plan
We are happy to be offering retiree dental coverage through Delta Dental. Please review the description of benefits and costs before making your decision. Coverage will be provided at 100% for preventive treatment, 75% for basic care and 50% for major services. The annual maximum is $1,000. You can roll over a portion of your unused spending to increase your maximum benefit limit next year and beyond. Please review the Delta Dental Rollover Max brochure for full details.

Important Notes:
- To elect coverage for you and your dependent(s) you must elect a 'family' plan.
- The monthly cost for individual coverage is $66.77. The monthly cost for family coverage is $180.24.
- You will be direct-billed for the dental coverage by CROSBY BENEFITS SYSTEMS, INC.
- Once enrolled in the dental plan you will receive a separate identification card from Delta Dental.
