Immunization Requirements

 

All newly accepted students are required to submit immunization documentation by the deadlines listed below:

Dental IS program - February 1st
DPG,MS,GPR programs - April 13th
DMD program - June 1st

Antibody Titer (blood work) Laboratory reports must be submitted with other immunization documentation.

The following immunization documentation is required as a condition of matriculation and must be kept up to date:

Tetanus Diphtheria Acellular Pertussis (Tdap): One dose of the onetime adult Tdap vaccine, in lieu of Td booster. The Tdap vaccine was licensed in 2005. Wait to receive the onetime Tdap vaccine if current Td booster date is less than 2 years old.

Measles, Mumps and Rubella (MMR): Two doses of measles, mumps and rubella vaccine or positive antibody titers.

Tuberculosis Test: Mantoux test given within one year prior to start date and updated annually. If tuberculin positive, documentation of a past positive test must be submitted. A chest x-ray or QuantiFERON-TB Gold Test report, dated within one year of the student’s start date, must also be submitted.  (A history of BCG vaccine is not acceptable as proof of being tuberculin positive. You must provide documentation of a past positive tuberculosis test in addition to a chest x-ray or QuantiFERON-TB Gold test report within one year prior to start date.)

Varicella (chicken pox): Physician verification of year of disease, two doses of varicella vaccine, or a positive antibody titer.

Hepatitis B:Three doses of hepatitis B vaccine and positive surface antibody titer. Dental students must be tested for presence of infection (HBsAg) prior to vaccination or if restarting the series, and testing for immunity (anti-HBs) within six months following the 3 doses.

The hepatitis B vaccine is a three-dose series, which takes six months to complete.  For students who have not already started the series, documentation for two doses of the vaccine must be submitted by deadline.

Meningococcal: One dose of vaccine within the past five years of start date or completion of State Waiver Form.
