To apply for insurance coverge, the application: Tufts Health Plan Member Enrollment Form, must be completed and submitted to the SAHA Office. You can print the application and instructions from the links below. However, to receive the complete Information Packet explaining the benefits, call the SAHA Office to request a packet by mail, or stop by the Boston Office at 200 Harrison Avenue, Room 407. On the Grafton campus, go to room 309-B in the Administration Building.
