Please answer each question thoroughly yet concisely, making sure to provide only pertinent information. IF YOU NEED TO PROVIDE MORE INFORMATION, WE CAN ONLY ACCEPT ONE ADDITIONAL PAGE PER PETFAX.
You can mail us pertinent video clips that illustrate the problem behavior if you have them.
Submit a completed Consultation Request Form and Behavior Fax Sheet by email to ronni.tinker@tufts.edu or fax them to Ronni Tinker’s attention at 1-508-839-8734. You can transmit to PETFAX 24 hours a day, 7 days a week. If you have any problems transmitting your fax, please call us at 1-508-887-4640.
